Palliative Care GP Home Visit Service in Wide Bay

⁠⁠⁠Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is inviting Expressions of Interest (EOIs) from general practices, in identified areas of Wide Bay, to provide at-home palliative care services.

Eligibility: to be eligible to submit an EOI general practices need to be located within one or more of the following four Statistical Area Level 2 (SA2 2016) area groupings:
Bundaberg SA2 areas:

• Bundaberg North; Gooburrum
• Bundaberg East; Kalkie
• Walkervale; Avenell Heights
• Svensson Heights; Norville
• Branyan; Kensington

Bargara SA2 area:

• Bargara; Burnett Heads (includes Elliot Heads)

Bundaberg Region; South SA2 area:

• Includes Childers and Woodgate

Hervey Bay SA2 areas:

• Urangan; Wondunna
• Pialba; Eli Waters
• Torquay; Scarness; Kawungan
• Point Vernon
• Craignish; Dundowran Beach
•  Booral; River Heads

Maryborough SA2 areas:

• Maryborough
• Tinana
• Granville
