Western Sydney Primary Health Network (WSPHN) is seeking proposals that address the priorities for Older Persons Mental Health and provides support for residents of aged care facilities within Western Sydney. The activity will work with clients that reside in the Western Sydney Local Health District.

WentWest is intending to provide funding to one or more providers to deliver this activity, with no option of sub-contracting. However, we encourage the successful organisation/s to develop partnerships and referral pathways with services to provide support for the clients.
Proposals must directly address Older Persons Mental Health and barriers faced by the residents of aged care facilities in the western Sydney catchment; such as the projected increase of people living with dementia in Western Sydney, insufficient understanding of cultural differences in regards to palliative/end-of-life care, and the sub-optimal screening and care planning for older people.
