Holistic End-of-life Care for Older People in the Community

⁠⁠⁠Brisbane South Primary Health Network (Brisbane South PHN) is seeking responses to this Request for Proposal (RFP) from eligible organisations to design, deliver and evaluate holistic end-of-life care options focussing on older people living in the region.

The services being procured will form part of a broader program of work aiming to improve integration and coordination of end of life care.
It is important to note that while sometimes used interchangeably, the term 'end of life care' differs from 'palliative care'.

For the purposes of this document the following definitions will apply:
The Australian Commission on Safety and Quality in Health Care describes 'end-of-life care' as the care delivered to people who are likely to die within the next 12 months, while palliative care may be needed over an extended period.

All proposals should demonstrate a strong evidence base, robust clinical governance mechanisms and relevant industry experience. These factors will be assessed when evaluating each proposal.

This RFP provides a framework to develop service solutions for the needs of people with a life-limiting illness, their families and carers. This includes plans to manage the establishment and ongoing development of the service. Consideration should be given as to how the model will ensure equity of access across the region, and manage service demand and prioritisation to ensure the people with the greatest need gain timely access to the service and supports.

The purpose of this RFP is to assess the ability of applicants to provide the services as specified in this document. An overview of Brisbane South PHN requirements in relation to this RFP is set out in Section 2, Service Specifications of this RFP.
This RFP provides applicant/s with advice and guidance about submitting a proposal. Applicant/s are advised to carefully review the instructions and selection criteria prior to submitting a proposal and budget.
