South Western Sydney LHD are seeking submissions from suitabliy qualified vendors for the provision of Residential Transitional Aged Care (TACP) Services. The Residential TACP Services provides short term support and active management for older people at the interface of the acute/sub-acute and Residential Aged Care sectors.
