Tender Description
In 2018, Darling Downs and West Moreton PHN in partnership with other key stakeholders in the Toowoomba region, conducted a pilot project to reduce preventable emergency department presentations by diverting patients to Primary Care when deemed appropriate. Data from the final report indicated that the desired project objectives were not achieved.
The aim of this research is to recommend alternative effective and efficient primary care models aimed at reducing non-urgent emergency department presentations that are achievable and appropriate for the Toowoomba region.
