Care Navigation Pilot

⁠⁠⁠This request is for suitably experienced and qualified Organisations to submit responses to conduct 12-month pilots of Care Navigation Services in Cessnock, Taree and Tamworth. To enable scalability, HNECC is also seeking to commission a Care Navigators Training Package that is modifiable according to the learnings from these pilots.

Care Navigation involves engaging with clients on a one-to-one basis to signpost / connect them to health, social and community services and provide education to navigate the health system. Care Navigation services aim to improve vulnerable and / or disadvantaged individuals':

* Ability to navigate access to the right services at the right time (health, social and community)
* Ability to overcome barriers to accessing care
* Health literacy and self-management
