WSPHN is seeking proposals for innovative projects to support healthy ageing for older people in western Sydney. Priorities have been jointly developed with the Western Sydney Local Health District (WSLHD) as part of the Western Sydney Integrated Health Partnerships Framework (2017-2020) and align with the WSPHN needs assessment.
The proposals should focus on the vulnerable elderly population who may not usually access programs and services and set to address at least one of the priorities below:
(a) Access to Healthy ageing and falls prevention programs
(b) Service Navigation and Advocacy
(c) Build Residential Aged Care Workforce Capabilities
