Deliver E-Referral Smart Form Capability Between Clinical Information Systems and My Aged Care

⁠⁠⁠We are seeking a Seller to co-develop, pilot and launch an e-Referral solution to allow health professionals to refer patients to My Aged Care directly from their clinical information system.

Ideally, the Seller brings expertise in smart form solutions and existing working relationship with multiple medical software vendors.

The Department is seeking an intermediary software vendor as a delivery partner to streamline the My Aged Care referral process for GPs and other professionals by developing and implementing a smart form solution that securely links, extracts, prepopulates and transmits patient data from health professional clinical information systems to My Aged Care.

Integrating referrals from health professional clinical information systems with My Aged Care is intended to:
* reduce the administrative burden associated with current referral pathways
* increase the number referrals to My Aged Care particularly by GPs among other health professionals
* encourage early intervention in line with a re-ablement approach
* enable faster assessments and more timely access to care
* result in more holistic assessments and service delivery better reflecting care needs and
* improve the quality and accuracy of data stored, which will improve data analytics and lead to better policy insights with improved health and ageing outcomes.

To develop, pilot and launch this capability, the Seller we partner with would be responsible for co-developing an integrated e-Referral solution that is ideally compatible with multiple health professional clinical information systems. The Seller may also be engaged to operate the solution for a specified period of time after the initial solution development and implementation.
