Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is inviting Tenders for the provision of the Integrated Team Care (ITC) Program in the Bundaberg Local Government Area.
The Integrated Team Care (ITC) program is a Commonwealth funded program providing chronic disease management care coordination and support to eligible Aboriginal and Torres Strait Islander clients. The Program was established to assist Aboriginal and Torres Strait Islander people (with complex chronic diseases), who are unable to effectively manage their conditions, through access to one-on-one assistance provided by Care Coordinators.
Eligibility:
* Bidders must not be a sole trader.
* Bidders must have current Public Liability Insurance of $20 million (or ability to obtain)
* Bidders must have current Professional Indemnity Insurance of $10 million (or ability to obtain).
* Bidders must have Workers Compensation insurance, as required by law.
Deadline for submissions: at 13:00 (1:00 PM) QLD TIME on Monday 17th December 2018, all responses must be fully received by this time.
