Request for Proposal for Chronic Disease Prevention and Management Initiatives in Western Sydney

⁠⁠⁠WSPHN is seeking proposals for projects to enhance Chronic Disease Prevention and Management across western Sydney.
The objective of this funding is to enhance services beyond what is currently funded by other sources such as MBS. Priority areas looking to fund include but not limited to are Primary Care Patient Services, Enhanced Health Literacy, Skills Development, Increased Access etc.

Since 2002 WentWest has been part of the western Sydney community, delivering support and education to primary care and working with key partners to progress the region's health system.

As the Western Sydney Primary Health Network, WentWest is focused on addressing both regional and national health challenges. Together with health professionals, partners from both the health and hospital sector, consumers and the broader community, WentWest seeks to identify gaps and commission solutions for better health outcomes.

Completed responses must be submitted via the online respond to tender form. Respondents will receive a Successful Submission Receipt timed and dated upon completion. Closing Date and Time: Submissions close at 4:00 PM AEDT on Monday 5th December 2018 and submissions must be fully received by this time.
