SWSPHN is seeking to commission a regional service provider, partnership or group of service providers, or consortium who will lead and provide a region-wide centre-based psychosocial service with a mobile outreach component to meet the psychosocial needs of people across South Western Sydney.
The proposed service will create a regional model for psychosocial support for people who have severe mental illness with associated reduced psychosocial functioning. The model will incorporate a number of centre-based services and have a mobile outreach component that will maximize accessibility. The centre-based service will value, facilitate and enhance social connection and friendships, led by the vision of: 'The place to go, where people know my name'. The consumer experience will be central to its delivery.
The psychosocial service will be available for drop in access, and also provide a number of structured programs and supports including: a group-based program and education; social activities and events; in-reach and outreach programs; psychoeducation; individual supports; and capacity building.
