COORDINARE - South Eastern NSW Primary Health Network (SENSW PHN) invites expressions of interest (EOIs) from suitable organisations or collaborations to apply for funding to assist in strengthening integrated palliative and end of life care service capacity in the community.
Through this funding, SENSW PHN seeks to support improved capacity, coordination and partnerships between providers of palliative and end-of-life care within the South Eastern NSW region. Ultimately, the funding will support improvements to and delivery of high quality palliative and end of life care in the community and an optimal end-of-life experience for individuals, their families and carers.
Organisations involved in community palliative and end of life and end-of-life care delivery within the South Eastern NSW region. This includes:
* General practices and other primary care providers
* Community pharmacies
* Community health providers (non-government funded)
* Aged care providers
Submissions from consortia and partnerships are strongly encouraged.
A total pool of funding up to $500,000 ex GST will be dispersed through this initiative.
