Supply of One (1) ENT Surgery Microscope to SCGH and One (1) ENT Surgery Microscope to RPH

⁠⁠⁠This Request is for the supply, delivery, installation, commissioning and maintenance of two (2) Ear, Nose and Throat (ENT) Surgery Microscopes: one (1) for Sir Charles Gairdner Hospital (SCGH) and one (1) for Royal Perth Hospital (RPH).

As a minimum, the scope of this purchase shall include:
* supply, delivery, installation and commissioning of the offered ENT Surgery Microscopes;
* standard accessories necessary for the complete and proper functioning of the offered ENT Surgery Microscopes;
* allowance for the provision of consumables for a period of up to ten (10) years;
* minimum twelve (12) month warranty;
* options for post-warranty service and maintenance for up to nine (9) years; and
* provision of in-service, education and training for the clinical users / operators, including the correct and efficient operation of the equipment.

A separate Head Agreement, having the following contract numbers, will be entered into by each Contract Authority:

* NMHS20171234 (SCGH); and
* EMHS20171649 (RPH).

The Head Agreements established by this Request will constitute a standing offer, whereby the Customer can place orders for consumables, training and/or maintenance options offered on an as required basis during the term of the Head Agreement.

The Contract Authorities reserve the right to award different contractors for their respective requirements.
