Request for Proposals - Alcohol and Other Drugs Relapse Prevention (Aftercare) Service

⁠⁠⁠Healthy North Coast Ltd, trading as North Coast Primary Health Network (NCPHN), is seeking proposals from organisations and/or consortia to develop and implement a community based relapse prevention service model in communities across NSW North Coast.

Specifically, the integrated model will focus on the establishment of person-centred relapse prevention services to support people, in the region, in recovery from alcohol and or drug dependence in the twelve months following treatment. The level and type of support will be tailored to individual client needs.

Engagement will be by way of a contract between NCPHN and a provider for a term of 12 months.
