Tender Description
Brisbane South PHN invites eligible organisations to submit a proposal to conduct an evaluation of the Optimal Care quality improvement program. This program is designed to facilitate best practice management of chronic disease in general practice by supporting practice staff (clinical and non-clinical) to implement quality improvements in their practice. General practice data is used as a guide to measure progress of the improvement over time.
Funding is provided for the evaluation period to be conducted between May and June 2018 with the final report submitted on 25 June.
The evaluation will be conducted in full partnership with the Brisbane South PHN.
