Central and Eastern Sydney PHN (CESPHN) is requesting proposals for the provision of Lifestyle Modification Programs for Adolescent Weight Management within the Central and Eastern Sydney PHN region especially in the areas with high overweight and obesity rates. Those areas include some suburbs in the former Canterbury LGA and Eastern Suburbs South.
The objectives of this project are to:
* Provide 200 or more adolescents with an evidence based program and/or interventions facilitated by qualified allied health professionals for management of overweight or obesity to achieve health equity; and
* Increase the population's access to programs and/or interventions that improve lifestyle risk factors.
