The Darling Downs and West Moreton Primary Health Network (DDWMPHN) Health Needs Assessment (HNA) (November 2017) outlines priorities in the areas of Aged Care and Chronic Disease. The need for a home outreach care activity was identified to respond to non-acute nursing care in the home over and above what is currently provided through HACC, facilitating care at home for people who otherwise may require social admission to hospital.
Consultation with key stakeholders identified the need to establish in-hours and after-hours service for people who require palliative care residing in their own home. This activity will deliver services to patients meeting eligibility criteria, and will assist them to be able to receive end of life care in their home instead of having to be moved into alternative care where appropriate. This will reduce the need to access an emergency response for non-acute situations with minimal disruption to the patient.
Implementation of this activity will create a service that incurs no-out-of-pocket expense to eligible patients. The patient will remain the patient of their referring GP or Specialist during their treatment by a palliative care nurse who will maintain a high level of communication with the patient’s GP.
Care will be provided in regular business hours and will include a funded afterhours component for the provision of required nursing care such as treatment of breakthrough pain.
