Tender Description
The objective of the tendering is to identify a suitably qualified service organisation which is committed to work within an TACP integrated care model programme in partnership with Western NSW Local Health District.
The programme is called Transitional Aged Care Program (TACP) and is a joint Australian government and NSW Ministry of Health program. The programme provides a mix of facility and community based transitional care to provide time-limited services with a rehabilitative focus to improve the transition from hospital to home for older people.
