NQPHN seeks to establish a bank of consultants and contractors to support the delivery of the readiness program, including: Preparation and delivery of Readiness Workshop modules.
Tailored supports for participating practices wishing to improve their readiness to become a designated Health Care Home.
Development of resources and tools to support HCH Readiness participants.
Program evaluation.
NQPHN's HCH Readiness Program is reflective of the HCH pilot program. While the exact deliverables for consultants are not known at this point in time (as the HCH pilot guidelines are yet to be released) there is a level of confidence that the clinical and administrative protocols will accord with those foreshadowed in the information provided to prospective pilot practices.
As such, and given the emergent nature of the HCH initiative, NQPHN has received Departmental approval to establish a bank of consultants with the requisite mix of expertise to provide tailored and flexible supports to practices participating in our HCH Readiness Program. Supports offered by consultants will need to be articulated with the suite of Readiness Workshops being planned. Where appropriate, consultants might also be engaged to deliver discrete workshop modules.
Expressions of Interest close at 2pm on the 18th June 2017 and submissions must be fully received by this time.
