North Coast Primary Health Network (NCPHN) is seeking Proposals, under its After Hours Program, for Projects which will lead to a reduction of potentially preventable hospital admissions (PPH) across the NCPHN high priority areas of Kempsey, Nambucca /Macksville, Grafton and Murwillumbah, or across the whole of the NCPHN footprint. NCPHN encourages innovative and flexible models and approaches from individual clinics, joint ventures, partnerships and/or consortia between organisations delivering services in line with the key objectives of the After Hours program.

Please note that an Information Briefing Session for Participants is scheduled to be held at 2:00 p.m. on 3 May 2017 at various NCPHN locations, and details regarding this Session are provided in Section C of the RFP documentation.
