SMHS seeking feedback from industry to improve the Application Guideline and Response document prior to release for submissions.
The aim of the Fremantle Street Doctor Pilot Sustainable Delivery Model and Mobile Health Clinic Services (FSDMHC) Grant is to provide a free, (Medicare bulk billed, no charge to patients) visible, easily accessible, culturally appropriate and non-judgemental mobile medical service for members of street based and disadvantaged populations in the South Metropolitan Health Service (SMHS) catchment of Fremantle and surrounding areas. The aim is also that services are delivered in a sensitive, confidential and caring manner by the FSDMHC and offer flexible and local service delivery at pre-determined sites encompassing all aspects of clients’ physical, mental and social health to a client base who would otherwise not seek care at GP Practices. FSDMHC clients are referred to suitable GP Practices and other providers where possible for ongoing long term management of health issues.
The Grant specifications will have a dual purpose for the duration of the Grant period:
1. Develop Sustainable Street Doctor Service Delivery Model (SSDSDM) - For the market to develop and pilot a sustainable service delivery model for FSDMHC Service, to cease/reduce reliance on State funds.
2. FSDMHC Service - To fund the financial shortfall between the Medicare rebate and the cost of providing the FSDMHC service to Fremantle and surrounding areas within the SMHS catchment area.
