Integrated Team Care

⁠⁠⁠Murray PHN invites expressions of interest from health service providers for the commissioning of Integrated Team Care (ITC) teams, commencing from 1 January 2017.

ITC teams work across mainstream and Aboriginal health services to improve access and coordination of care for Aboriginal and Torres Strait Islander people with chronic disease. Located across the Murray PHN catchment area, each ITC team will include:
* Indigenous Health Project Worker who supports improved collaboration and integration between health services.
* Care Coordinator a qualified nurse or Aboriginal Health Worker.
* Outreach Worker providing practical assistance to help patients access health services.
