Do you have an idea to improve access to after hours primary health care in the Tasmanian community?
Primary Health Tasmania is seeking tender applications from individual providers and/or organisations who may wish to:
a. work with Primary Health Tasmania in developing an innovative model of care, or
b. deliver an innovative model of care in the community.
Funding is being provided to improve access to primary health after hours care for Tasmanians, with a particular emphasis on vulnerable people. This includes the elderly, people with complex health conditions, people on low incomes, palliative care and house-bound people. There is also an emphasis on areas with little or no after hours' general practice coverage.
Successful applicants will be funded for the 2016-2017 financial year. Key Performance Indicators will be used to measure services and outcomes.
