Delivery of Rural and Regional Diabetes Education and Support Services

⁠⁠⁠Delivery of rural and regional diabetes education and support services in a primary care setting in North and North West Tasmania.

Primary Health Tasmania is seeking tender applications from suitably qualified service providers, either independently or jointly (e.g. in group/consortium arrangements), describing how they will work efficiently and effectively to deliver evidence based, type 2 and non-complex type 1 diabetes education and support services, across North and North West Tasmania.

Successful applicants will be funded from 1 July 2016, and will be offered a service agreement for 6 months. Key Performance Indicators will be used to measure services and outcomes.
