Northern Sydney Local Health District (NSLHD) is seeking to contract with a Private Health Service Provider for the delivery of a GP based Chronic Disease Care Coordination pilot Program over 3 years, which aims to better coordinate care for chronic and complex patients.

The vision is for the Program to establish physical and virtual linkages between patients and their carers, General Practice, hospitals and other health and home care providers. The Program objectives are to achieve an improved health system experience, reduced hospitalisations and time in hospital, leading to an overall improved quality of life.

Through this Request for Tender (RFT) process, NSLHD is seeking specific information from potential Respondents which demonstrates their capability of providing such a Program with strong potential for transferability and scalability across the entire LHD and potentially other LHDs. Importantly, the LHD views the Program as an opportunity to partner with Private Health Funds to joint-fund this initiative.
