FCMU2015.16-003A is the first stage of a two stage procurement process to engage Service Provider/s to deliver the Health for Life! program.  An opportunity exists for service provider/s to access Department of Health funding for the provision of the new Health for Life! Program.  The Program is a statewide, integrated risk assessment and lifestyle modification program.

Delivered by appropriately qualified and trained staff, the program will make it easier for at least 10,000 Queensland adults who are assessed as being at high risk of developing type 2 diabetes, cardiovascular disease (CVD), some lifestyle-related cancers and obesity, to understand the risk factors most applicable to them and complete the Health for Life! program.

Program objective are:

* To effectively identify people at high risk of developing chronic disease, and refer them to an appropriate lifestyle modification intervention based on their level of risk
* To increase health literacy and the capacity of program participants to adopt and maintain positive lifestyle changes to manage their risk factors.
* To improve community awareness, knowledge and attitudes about chronic disease risk factors and how to make positive lifestyle choices.

Health for Life! will be based on the Victorian Department of Health’s Life! Helping You Prevent Diabetes, Heart Disease & Stroke program. Information about the existing Victorian Life! program is available from http://www.lifeprogram.org.au/.

The Queensland Health for Life! program will require culturally tailored approaches across service modalities to support targeted population groups including Aboriginal and Torres Strait Islander and Culturally and Linguistically Diverse communities.
