Request for Tender (RFT) is to contract an Approved Aged Care Provider who is committed to work within an integrated care model with WNSWLHD to provide Community Transitional Aged Care Program for thirty nine (39) flexible care places for eligible older people who are resident in the local government areas within the WNSWLHD.
