Coast Hospital and Health Service deliver Transition Care services to clients within the Gold Coast community. The program operates under the Aged Care Act 1997 and the Transition Care Program Guidelines. As part of the program, services can be provided in either a community or residential environment.

To assist with the provision of residential based services, Gold Coast Hospital and Health Service are issuing an Invitation to Offer for residential care facilities to assist in the provision of Gold Coast Residential Transition Care services.
